TF-CBT: Implementation Challenges and Benefits in a Community-Based Clinic Using Telehealth/In Person Service

It has been reported that 25% of children and adolescents in the community experience at least one traumatic event in their lifetime. Such trauma includes life threatening accidents; abuse; maltreatment; family or community violence; assaults; and disasters (Ko, et al, 2008, p. 397).
Although traumatized children may be able to bounce back quickly, traumatic events still tend to have profound effects on a child’s and adolescent’s development. Exposure to traumatic events also increases risk factors associated with academic performance; increases the use of health and mental health services; and increases involvement with child welfare and juvenile justice systems.
TF-CBT is a short term evidenced based treatment. The treatment model integrates trauma-sensitive intervention with cognitive-behavioral strategies. Its goal is to correct maladaptive or unhelpful beliefs related to the abusive experience as well as decrease triggers that cause emotional dysregulation. Children are encouraged to share their story, and are taught cognitive strategies so that they can complete the trauma narrative with less anxiety. They are gradually exposed to the event through cognitive strategies and relaxation techniques.
This workshop will provide an overview of using TF-CBT and other techniques for children exposed to traumas. We will explore in depth symptoms that children exposed to trauma typically have and how to identify if these sxs are interfering with normal functioning. Providing some psychoeducaiotn about different types of traumas will be included, for example sexual/physical abuse, domestic violence and community violence as well as other traumas often not identified by experts as traumas.
Learning Objectives
As a result of this workshop, participants will:
- Provide an overview of basic TF-CBT techniques and further study for participants.
- Review symptoms of trauma in children and how to assess if a child needs therapy or a short intervention.
- Provide resources for further training and assessment of trauma.
- Discuss challenges in an outpatient setting given the new telehealth services as well as engaging parents to participate in this therapy.

Alice Greenfield, LCSW
Behavior Health Supervisor of the FRIENDS Clinic/VNSNY, Mental Health Outpatient Treatment and Rehabilitative Services (MHOTRS)
Alice has worked with children and adolescents for over 25 years. Prior to coming to the FRIENDS clinic she worked as a therapist at New York Presbyterian Hospital’s Child Psychiatry Clinic for 25 years. Currently Alice is the Behavior Health Supervisor of the FRIENDS clinic, which provides clinical psychotherapy to children and adolescents and families in the community. The program provides therapy to those children who have been diagnosed with mental health issues such as ADHD, Depression, Anxiety and PTSD. The program also provides medication management to their clients and they see adults who have children attending the clinic.
Alice served as the co-chair for the BBC (Bronx Borough Based Council) from 2017 to 2022. The BBC is an organization that promotes and works with families, youth, government agencies and providers to improve services in child and family servicing systems and identify solutions for families being served by more than one agency. BBC meets monthly in the Bronx.
Alice has supervised social work interns (MSW) and collaborated with other disciplines such as psychologists, case managers, psychiatrists, and Nurse Practitioners for over 30 years. Her main interest and specialty is providing the best trauma care she can for children, adolescents and families exposed to trauma. She has lectured at Columbia School of Social Work and NYU School of Social Work on Trauma-Focused Cognitive Behavior Therapy (TF-CBT), which is an evidenced based trauma therapy for children exposed to violence, domestic violence and sexual abuse. She presented in May 2017 at the Annual NASW NJ Conference on Practicing Cultural Competence with Children Exposed to Trauma. She presented the workshop Incorporating Therapy Technique to Children Exposed to Trauma at NYU Silver’s CE Series in November 2019. She also presented at a Town Hall in August 2019 entitled How Bronx Schools Can Address Childhood Trauma. This program was composed of Assembly Members as well as clinical professionals and community members convening to learn about ways to support with childhood trauma. Alice also has presented on TF-CBT for VNS’s Behavioral Health Learning Institute on the DSM 5 as well as on Trauma Informed Treatment. She is also part of the Trauma Informed Care Training Group that serves at VNS to support all staff who have clients exposed to trauma. This group meet bimonthly and provides psychoeducation as well as support to their staff.

Step 2: On the right hand side of the page, you will see a hyperlink that says "Register" (located below the 'Log In' button). Click on "Register"
Step 3: Fill in all of the fields and click the "Register" button.
Step 4: Once you complete the user profile, you will have access to the Online Portal. For information on how to apply for a program or register for a particular event, please see the information below.
How to Apply or Register through the Online Portal
Step 1: If you are not already logged in, go to https://sswforms.es.its.nyu.edu/ to log into the Online Portal. On the right hand side of the page, enter your email address and password in the appropriate fields. Click the "Log In" button.
Step 2: You will be taken to the "All Events & Programs" section of the Portal. From the list of available programs, select the program you are interested in attending.
Step 3: For Post-Master's Certificate Programs, you will need to click on the "Apply" link for the program you are interested in. You will be taken to an application for the program you have selected and asked to complete the form. Once you have entered all of the appropriate information, click the "Submit" button to submit your application. We review applications on a rolling basis and respond with a decision within 2 - 3 weeks of receipt.
For all Events, you will need to click the "Register" link for the event you are interested in. You will be taken to a registration form. Once you have entered all of the appropriate information, click the "Submit" button and you will be taken to a secure credit card payment page. Complete the payment page and click "Submit" in order to fully register for an Event. A successful registration will yield a confirmation email sent to you. If you do not receive a confirmation email to your inbox within an hour, make sure to check your spam folder before reaching out to the Office of Global and Lifelong Learning, as our messages can occasionally be filtered into spam folders. Cancellations and Refunds
If after registering, you determine that you can no longer attend this event, The Office of Global and Lifelong Learning will issue refunds on the following basis:
- Refund requests made on or before June 7, 2024 (by midnight): full refund
- Refund requests made on or before June 11, 2024 (by midnight): 50% refund
- Refund requests made on or after June 12, 2024: no refund
If this event is cancelled, all registrants will be fully reimbursed. To withdraw from and be reimbursed for this event, please complete the event withdrawal form.
